“Today we have more data, that's hopeful, but unlikely to lead to meaningful coverage, because the academic researchers chose to study something. I bring you: IV ketamine. For bipolar depression.”
The KET-BD randomized trial (IV ketamine vs midazolam, 3 Ontario sites, July 2022 to November 2025) showed a significant MADRS benefit in treatment-resistant bipolar depression; the author forecasts this academic data is unlikely to lead to meaningful payer coverage of IV ketamine for bipolar depression.
Intake
- EventA proposition written from a template on the asset, with a resolution criterion and a named public source, before the outcome is looked up. The same event resolves every person's items on it. Method ›
- A US health plan covers intravenous ketamine for bipolar depression
- AreaOne of five topic classes: regulatory decisions, clinical trial outcomes, company and market outcomes, payer and policy, practice adoption. Every event has one area. Method ›
- Payer and policy
- DeadlineThe date by which the event must happen for the item to be true. It comes only from the person's words through the anchor table. Method ›
- None stated · undated panelItems with no deadline in the person's words, scored on a uniform 24-month window from the statement date. Part of the headline panel since rules 1.1.0; also shown on its own. Method ›, 24-month window to 3 Sep 2028
- Probability
- 0.30 LexiconThe fixed five-bin table that turns a phrase into a probability. The strongest phrase in the claim sentence decides; coders never read tone. Method › · binOne of five lexicon steps: A 'will' 0.90, B 'probably' 0.70, C 'may' 0.50, D 'unlikely' 0.30, E 'never' 0.10. A stated number goes to the nearest bin for calibration only. Method › D · “unlikely to lead to meaningful coverage”
- Base rateThe probability a published external study gives an event of that class and stage, fixed at intake. Halved when the item window is shorter than the class median time to decision. Never learned from the ledger's own items. Method ›

- Coder BCohen's kappa: agreement between two coders beyond chance, from -1 to 1. Reported for admit, event, deadline, bin and asserts from the second-coder match test. Method ›
- admittedA statement that passed intake: sincere, the person's own claim, forward-looking, inside the five areas, and not excluded by a reason code. Only admitted statements become items. Method › · event E-0131 · deadline none · bin B
Registry entry
A US health insurer, health plan or public program publishes a coverage policy that covers intravenous ketamine for bipolar depression.
TRUE on the effective date of a published medical or coverage policy, fee schedule entry, national coverage determination, local coverage determination or public policy bulletin from a US health insurer, health plan or public program (a commercial plan, Medicare, Medicaid or the VA) that lists intravenous ketamine as covered or medically necessary for bipolar depression. A policy that covers esketamine (Spravato), a policy limited to major depressive disorder, an individual case exception, coverage limited to a clinical trial, and a payment for the infusion visit without the drug do not count. FALSE at the deadline when no such policy is in force. Resolved on the payer's published policy bulletin and the CMS Medicare Coverage Database.
Resolution source: Payer medical policy bulletins; CMS Medicare Coverage Database
Readings: 'Meaningful coverage' is read as at least one published US health plan coverage policy for the indication; the forecaster names no payer, so any US health plan or public program counts, as in E-0116.; 'IV ketamine. For bipolar depression.' fixes the asset and the indication; esketamine is a different product and does not count.; The KET-BD trial result is the reason the forecaster gives, not part of the criterion.

Resolution
- Effective deadlineAn item enters the score only when its deadline has passed and the outcome is true or false. A claim already known true still waits for its deadline. Method ›
- 3 Sep 2028
- Outcome1 if the registry event happened on or before the item's deadline, 0 if it did not. Written by a resolver who never sees p. Method ›
- not occurred · checked through 14 Sep 2026
- Resolver note
- No US health plan policy that lists IV ketamine as covered for bipolar depression was found in force on 2026-09-14. Checked: UnitedHealthcare Community Plan CS2026D0069AA, effective 2026-05-01, calls ketamine injection investigational for psychiatric disorders, and names bipolar disorder; Excellus BCBS 3.01.13, effective 2026-01-22, calls it investigational for all diagnosis codes; Wisconsin Medicaid (ForwardHealth Update 2024-53, effective 2025-01-01) covers IV ketamine only for major depressive disorder and refuses prior authorization for any other off-label indication, so it is excluded; Kaiser Washington criteria, last reviewed 2025-12-02, are non-coverage and record no Medicare NCD or LCD. The Ket-BD RCT published in JAMA Psychiatry in 2026 has not yet produced a payer policy. This states nothing about later dates.
