“Anxious depression, and then up to 86, down, age 15 and more is coming, but we're not there yet.”
Psychiatry Boot Camp with Mark Mullen. Muir summarizes current FDA TMS clearances (MDD, OCD, smoking, anxious depression, ages 15 to 86) and says more indications are coming.
Intake
- EventA proposition written from a template on the asset, with a resolution criterion and a named public source, before the outcome is looked up. The same event resolves every person's items on it. Method ›
- FDA authorizes a TMS device for a new indication
- AreaOne of five topic classes: regulatory decisions, clinical trial outcomes, company and market outcomes, payer and policy, practice adoption. Every event has one area. Method ›
- Regulatory decisions
- DeadlineThe date by which the event must happen for the item to be true. It comes only from the person's words through the anchor table. Method ›
- None stated · undated panelItems with no deadline in the person's words, scored on a uniform 24-month window from the statement date. Part of the headline panel since rules 1.1.0; also shown on its own. Method ›, 24-month window to 23 Mar 2028
- Probability
- 0.90 LexiconThe fixed five-bin table that turns a phrase into a probability. The strongest phrase in the claim sentence decides; coders never read tone. Method › · binOne of five lexicon steps: A 'will' 0.90, B 'probably' 0.70, C 'may' 0.50, D 'unlikely' 0.30, E 'never' 0.10. A stated number goes to the nearest bin for calibration only. Method › A · “more is coming”
- Base rateThe probability a published external study gives an event of that class and stage, fixed at intake. Halved when the item window is shorter than the class median time to decision. Never learned from the ledger's own items. Method ›
- None in the table
- Tags
- retrospective
- Coder BCohen's kappa: agreement between two coders beyond chance, from -1 to 1. Reported for admit, event, deadline, bin and asserts from the second-coder match test. Method ›
- admittedA statement that passed intake: sincere, the person's own claim, forward-looking, inside the five areas, and not excluded by a reason code. Only admitted statements become items. Method › · event E-0019 · deadline none · bin A
Registry entry
The FDA grants a 510(k) clearance, De Novo authorization or PMA approval for a transcranial magnetic stimulation device for a disease or condition beyond those authorized for any TMS device at the statement date.
TRUE on the decision date in the FDA 510(k), De Novo or PMA database of a marketing authorization, dated after 2024-01-04, for a transcranial magnetic stimulation device whose indications-for-use statement names a disease or condition that no TMS device was authorized for on 2024-01-04. Conditions already authorized at the statement date (major depressive disorder including treatment-resistant depression, anxious depression, smoking cessation, obsessive-compulsive disorder, migraine for single-pulse TMS) do not count. A new population or protocol within an authorized condition, a breakthrough designation, an investigational device exemption or a non-US authorization does not count. Resolution source: the FDA 510(k) Premarket Notification database, with the De Novo and PMA databases for other pathways. FALSE at the deadline when no such authorization appears there.
Resolution source: FDA 510(k) Premarket Notification database; De Novo and PMA databases for other pathways
Readings: 'indications' = FDA-authorized indications for use of TMS devices. The forecaster lists the four conditions he treats as current; they are the FDA-cleared psychiatric and addiction indications at the statement date.; 'more indications' = at least one disease or condition new to TMS as a device class; one authorization for one new condition suffices. Any TMS manufacturer counts, since the forecaster names none.; A population expansion or a protocol change within an already authorized condition is not a new indication; a listed condition under another name (MDD for TRD) is not new.; TRD counts as MDD.; The forecaster counts anxious depression as its own indication, so a new patient population for a listed disorder counts as a new indication.

- State
- Known trueAn item whose event has already happened but whose deadline has not passed. Shown as 'known true, enters on <deadline>'; pending in every metric until then. Method ›
- Effective deadlineAn item enters the score only when its deadline has passed and the outcome is true or false. A claim already known true still waits for its deadline. Method ›

- Outcome1 if the registry event happened on or before the item's deadline, 0 if it did not. Written by a resolver who never sees p. Method ›

- Resolver note
- FDA 510(k) K260402 (Wave Neuroscience MeRT System) has decision date 2026-06-03 under 21 CFR 882.5802 with new product code SIG; the indications for use name an adjunct for treatment of adult patients with PTSD. PTSD was not an authorized TMS indication on 2024-01-04 (authorized: MDD/TRD, anxious depression, OCD, smoking cessation, sTMS migraine), so TRUE on 2026-06-03. Earlier check: every TMS 510(k) decided 2024-01-05 to 2026-06-02 carried product code OBP (MDD) or QCI (OCD); the adolescent MDD clearances (Neuronetics K231926, 2024-03-22; BrainsWay K251391, 2025-11-07) and accelerated protocols are population or protocol changes that the criterion excludes; no TMS PMA and no other new TMS product code exist. One registry reading says a new population counts; the criterion as written excludes it and governs here. Under that reading the date would be 2024-03-22 (K231926).

Adversarial recheck
escalated (criterion): Verified K260402 (Wave Neuroscience MeRT, new product code SIG, 21 CFR 882.5802, decision 2026-06-03, PTSD) in the FDA 510(k) database and openFDA. openFDA lists 24 OBP (MDD) and 2 QCI (OCD) clearances from 2024-01-05 to 2026-09-13, no new QMD or OKP entries, and SIG as the only new TMS product code. Population and protocol clearances (NeuroStar adolescents K231926 2024-03-22; Magstim K243869; MagVenture K251119; BrainsWay K251391; Nexstim K252358) are excluded by the criterion text. Magstim's 2025-07-09 'pain' clearance is peripheral nerve stimulation held since K992911 (2000), not a new TMS condition. So under the criterion as written 2026-06-03 is right. But reading 5 says 'a new patient population for a listed disorder counts as a new indication', which makes K231926 (2024-03-22) the first-true date, 26 months earlier and possibly inside deadlines the criterion text resolves FALSE. Criterion and readings contradict each other; a human must pick which governs.
