Footnote 12 explains that Schmaltz is a made-up drug combining IL-17 inhibitors (Taltz, Cosentyx) with a JAK/STAT modulator (Xeljanz), and that insurance currently will not authorize both at once because the combination is not 'medically necessary'. The author promises more on medical necessity in later chapters of the serial.
