'Gem #2', the mock 'Approve a Claim, Deny a Claim' program in which every in-network physician also acts as a conscripted claims reviewer. The author says such incentives would make everyone game the system to UHG's benefit, then adds that the current unpaid appeals burden is 'not actually more crazy'.
